A copy of each parent/guardian’s passport and valid documentation to reside in mainland China.
A copy of the student’s passport and valid documentation to reside in mainland China.
A copy of the student’s birth certificate.
One passport photo of the student.
A copy of the student’s official school reports, or Certificate of Attendance and Enrolment issued by the previous school, in English or Chinese, or a verified translation, from the previous two school years.
A copy of the vaccination card.
A copy of the medical report within the last three months from entry date.
(Items to be included in the medical report: medical history, physical examination, E.N.T. examination, teeth and vision examination, internal examination, blood examination, liver function examination, doctor’s diagnosis and suggestion, doctor’s signature and hospital stamp).
A copy of any detailed specialist reports if Special Educational Needs are identified in the application.
Signing a disclosure clause to ensure all student information relevant to the application is included and is accurate.
